LSS Ventura County operates and manages emergency assistance programs throughout Conejo Valley. We provide short-term emergency assistance, addressing the immediate needs of marginalized and underserved communities, while promoting a sense of self-reliance and community among all peaple.

CASE MANAGEMENT SERVICES

  • Housing Navigation Assistance

  • Security Deposit

  • Essential Household Items

  • Referrals to Potential Permanent Housing Opportunities

  • Case Management for six to twelve (6-12) Months After

  • Securing Housing

  • Referrals to Additional Resources

Process For Assistance

  • Meet HUD’s homeless/at-risk criteria

  • Medi-Cal coverage-HealthNet, Molina, Anthem, LA Care, GC

  • California ID, SSC, Insurance Card, Birth Certificate

Qualifications:

  • Coordinated Entry System (CES) Matching

  • Assist in Housing Navigation and Unit Viewings

  • LSSSC will Cover Application Fees, Security Deposit, Utility

  • Deposit Assistance

Housing Search:

  • Lease Signing

  • Landlord/Tenant Issues

  • Provide Additional Resources (food, clothing, education, etc.)

  • Monthly Case Management for Ongoing Support
