LSSSC Los Angeles County San Fernando Valley, based in Granada Hills, operates the CalAIM Community Supports program. Our program aims to help individuals and families who are homeless or at risk of homelessness improve Medi-Cal members’ quality of life and health outcomes. Additionally, we have partnered with Enhanced Care Managers (ECM) to coordinate comprehensive health and health-related care, covering physical, mental, dental care, and social services.

OUR SERVICES

The program’s key elements of our Housing Navigation services include:

Participant Screening and Housing Assessment: Once the participant is eligible and authorized by the MediaCal managed health plan, an assessment is conducted to understand tenants’ housing-related needs and barriers.

Individualized Housing Support Plan: A personalized plan is created based on the assessment, complete with measurable goals and strategies specific to each individual’s challenges.

Benefits Advocacy: Assistance is given in obtaining government benefits, such as SSI, that can aid in securing suitable housing by referring to services such as the Countywide Benefits Entitlement Services Team (CBEST).

Resource Identification: The program identifies additional resources for rent subsidies and covers various expenses, including security deposits, moving costs, adaptive aids, and environmental modifications.

Housing Search: The program and participants collaborate actively, searching for appropriate housing options based on eligibility.

Assistance in Securing Housing: Support is provided in completing housing applications and gathering required documentation like identification and rental history.

Move-In Assistance: Housing Deposits assist with identifying, coordinating, securing, or funding one-time services to new housing based on approval.

Housing Retention: Procedures and contacts are established to help participants maintain their housing.

Advocacy with Landlords: Communication and advocacy address participants’ needs and concerns with landlords.

Safety Assurance: Ensuring the safety and suitability of the living environment is a priority.

Reasonable Accommodation: Assistance is offered in requesting reasonable accommodation to ensure accessibility and comfort.

Landlord Engagement: The program educates and engages with landlords on behalf of or in collaboration with participants.

Ambassadors are short-term volunteers who open doors for our organization in the community, introducing new people at our bi-monthly Fostering Community Care event. These are one hour awareness events only (no solicitations are made) to expose people to the great work of LSS Community Care. To become an Ambassador here are your responsibilities:
  1. 1 – Attend at least one Fostering Community Care event during the calendar year
  2. 2 – Host or invite and bring 10-15 guests to one Fostering Community Care event within 3 months of your attending one yourself.
  3. 3 – Inform your invited guests of what the Fostering Community Care meeting will be about, and let them know that they will receive a follow up call from the organization
  4. 4 – Ambassadors will complete their commitment after bringing 10-15 persons within the 3 month time period. An Ambassador has the option to complete after one term, continue for another 3 month term or move into another role on the team.
